For standard requisitions for service providers (SP), we will need to set the vendor up in our vendor management system. In order to do so, we need a completed and signed W9 and contact information (name, email, and phone) to get a vendor ID created. After a vendor ID is created we can submit a PO requisition.
For the PO requisition, we’ll need the vendor to sign and return the following:
1. Complete, sign and return the attached Scope of Work document (this document must also be countersigned by the SPS department)
2. If the SP identifies as an Independent Contractor, complete and return the below certification form
3. Complete, sign and return the attached CU Service Provider Agreement (separate forms for Marketing and IT related projects) (This document will be countersigned by central CU Procurement. The department should NOT countersign this document)
4. If any work is done on CU property, we require a Certificate of Insurance (see sample below - MUST FOLLOW THIS FORMAT AND LIST CU AS ADDITIONAL INSURED)
5. Signed W-9 (or W-8 for International Vendors) on the latest version of the IRS Form (for new vendors)
6. Name, Email and Phone Number of Service Provider
7. Quote, Resume (or CV) and Brochure, where available
8. Sole/Single Source Justification or 2-3 bids

All documents should be submitted through our web portal using the Purchase Requisition Web Form.
Once we submit all the necessary documents to Procurement, it takes them about 2-4 weeks to review and approve a purchase order. We are able to then process the payment the same day the PO is issued and takes approximately 2-5 business days. You can also set up a direct deposit account online to speed up the payment process.
